**Due to rising duplicate order requests, all duplicate plaques must be placed between Oct. 1 - Nov. 30. One large batch order will be sent to our awards manufacturer after the Nov. 30 deadline. All duplicate plaques will be mailed in December. No orders will be accepted outside of the duplicate ordering timeframe.

KAB must receive payment before sending awards to our manufacturer. An invoice will be e-mailed to you within two business days upon form completion. Once payment has been received, we will submit your order to our manufacturer after the Nov. 30 deadline. You will then receive your award in December. Cost per plaque:

1st place (acrylic in frame) $70 each plus $12 for shipping PER award

2nd place (5" x 7" plaque) $20 each plus $12 for shipping 

Please list the award information EXACTLY as you want it to appear on the award. Post-production changes are at the cost of the person ordering the award.
